Biweekly Hours: 80.00

Shift: 8a-430p

Summary:

Helps patients with substance use disorders (SUD) and their families navigate successfully through the health care system by removing barriers to care, including providing access to resources, transportation, and support and enhancing communication with the health care providers. Treatment in the ED includes but is not limited to, regularly performing SUD assessments, linkages to ongoing treatment, linkages to community resources, educating patients, communicating with clinical and administrative staff, monitoring and reporting outcomes, and participation in other grant-related meetings and activities. Ensures follow-up regularly occurs with the identified patient population for up to 6 weeks or longer as necessary following ED discharge

Qualifications:

Required: High School diploma or passage of a high school equivalency exam. Good computer skills, including Microsoft Office software products. Demonstrated effective communication and interpersonal skills. Ability to work in a team environment. Culturally sensitive and comfortable working with a diverse urban population. Organized and detail-oriented. Able to work a flexible schedule, including some evenings or weekends. Reliable transportation to make patient home visits as needed. Ability to interact effectively with a wide range of cultural, ethnic, racial, and socioeconomic backgrounds.

Preferred: Bachelor’s degree in a related field. 5 years of medical health care experience. Experience in social/human services, criminal justice, or mental health/substance abuse. Bilingual or multilingual skills.

Physical Demands: May need to move around intermittently during the day, including sitting, standing, stooping, bending, and ambulating. May need to remain still for extended periods, including sitting and standing. Ability to communicate in face-to-face, phone, email, and other communications. Ability to read job-related documents. Ability to use a computer.
